Memory loss is never normal and should not be considered an inevitable part of aging. The Memory Loss Centers are designed to provide support to individuals with memory loss and the friends and family members who care for them. Our staff of compassionate experts is available to help you and your family cope with the special challenges of loving someone who is experiencing memory loss.
The Memory Loss Centers
The Memory Loss Centers are located at St. Michael Hospital (2400 West Villard Avenue) and St. Francis Hospital (3237 South 16th Street). To learn about services at either location, or to make an appointment, please call us at (414) 527-8298.
Services
At the Memory Loss Centers, services are provided by an interdisciplinary team which includes a physician, a psychologist, a registered nurse (RN) and a social worker. Additional consultation may be provided by behavioral specialists, dietitians, pharmacists, physical and occupational therapists and other health care professionals. Services include:
" Evaluation of suspected dementia
" Complete evaluation of patient and caregiver needs
" Linkage with community resources
" Dementia treatment and management
" Caregiver education and follow-up
" Support group
The Memory Loss Centers welcome inquiries on behalf of anyone who is experiencing a loss of memory or cognitive function. The Memory Loss Centers accept Medicare, Medicaid, self-payment, private insurance and some HMO's (with pre-authorization).
